I notice that on guide from SBS 2011 to 2019 the recommendation is to Migrate the CA however that step is not in the 2016 guide. Exchange is hosted on O365.
Questions:
-Is there a reason for above?
-Is this step only a recommendation?
-What happens if one does not perform migration

Not sure which guide you are looking but in chapter 16 of https://server-essentials.com/support/migrate-sbs-2011-to-windows-server-2016-standard-or-datacenter we also migrate the CA.
Migrating the CA is not needed when you don't use it but I would always back up the CA as described in the first steps and uninstall it from the SBS. That way we can always recreate it when needed. Most small businesses don't use the CA but if you do for example for file encryption or in a Radius server that hands out certificates for clients connecting to a wireless network that is secured with certificates you must migrate.
